Save time by filling out your paperwork in advance. All forms are available to download, print, and bring to your first appointment. 

Insurance Plans Accepted

We accept Medicare. We are working with a full range of private insurances. Please call our office to verify if your insurance is accepted. 

In the meantime, please contact our front desk at 662-469-5758 to verify coverage.

Frequently Asked Questions

Walk-ins are always welcomed. We also do recommend scheduling an appointment to minimize wait time. Same-day appointments may be available depending on the schedule. 

Please bring:

  • A valid photo ID
  • Your insurance card
  • Any medications or a list of medications you are currently taking
  • Completed patient forms

Yes, we offer virtual visits for certain conditions. Please call our office to find out if your concern can be handled via telehealth.

If you are more than 15 minutes late, we may need to reschedule your appointment depending on availability or we may attend to another patient to avoid any further delays.

Cancellation & No-Show Policy

We understand that life happens. If you need to cancel or reschedule, please contact us at least 24 hours in advance so that we may accommodate other patients. 

  • Missed appointments or same-day cancellations may be subject to a fee of around $25.
  • No-shows will be subject to a $50 fee.
  • Repeated no-shows may result in limited access to future scheduling.

     

Our goal is to provide timely care to all patients. We appreciate your understanding.

Consent Forms

We require written consent for the following services and procedures. These forms will be provided in-office or electronically before your appointment:

  • Medical treatment & evaluation
  • Telehealth consultations
  • PRP or injectable treatments
  • Medication management and controlled substances

If you have questions about any consent documents, we’re happy to explain them during your visit.
